Why Self-Advocacy Training
Self- Advocacy training and supports helps people build the skills they need to advocate for themselves. Self-advocacy training and support may help a person:

Build self-confidence in expressing themselves and making informed choices

Learn about the options available and how to navigate the service system
-
Develop skills to become more involved in the community and communicate their preferences.
Examples of when to explore self-advocacy training:

Does not express their preference, or looks to others to express their preference on their behalf

Wants to make changes to aspects of their life
-
Expresses frustration but is overwhelmed by or uncertain of their options
-
Aspires to build their network of support and become more involved in their community
-
Communicates through actions that they are seeking change
-
Moves to an individualized budget for their services.

Why Person Centered Planning
Person-centered planning is learning through shared actions. We facilitate the planning process to support the person and their team. However, planning is more than just making better paper; it’s about enhancing people’s lives. Plans evolve as people evolve, and as we learn through implementation.
Examples of when to explore person centered planning:

The person asks for it

The person’s voice is not being heard or his or her voice is not strong enough

The person or team feels “stuck” and needs more in-depth support to envision and articulate the person’s desired future

A person doesn’t know what he or she wants and doesn’t know the possibilities
-
A major change like transitioning out of school or out of the family home The person’s team is at odds/disagrees about future steps or next directions
-
The support planner and team are having trouble figuring out what is important to or for the person and the balance between important to and important for
-
Prior plans have not produced good/meaningful outcomes for the person
-
The team is stuck on implementation of the person’s vision or needs support in deepening the planning process with the person

Steps to Referral:

Case manager completes form
-
Our team will contact you to take the person's information.

Consultation will be scheduled with the person
-
Service needs and units will be established.

The Case Manager will submit a referral to Connectability.
-
The service agreement is approved service begin.
